ICIC16 – 16th International Conference on Integrated Care

The conference will be held in Barcelona from the 23th to the 25th of May and will bring together researchers, clinicians and managers from around the world who are engaged in the design and delivery of integrated health and social care. They will share experience and the latest evidence about integrating Public Health, Health and Social Care and the New roles and Possibilities for Hospitals, producing Positive and Curative Integrated Mental and Physical Care, mobilising key enablers like policy making and Mobile and Digital Health Solutions, and investment in an Integrated Care Workforce, clinical leadership and coproduction with individuals, careers, communities and populations.

Supporting integration through new roles and working across boundaries

In 2015, The King’s Fund was commissioned by NHS Employers and the Local Government Association to produce an independent report on boundary-spanning roles to support integrated care. The work aimed to identify examples of new roles being developed and an understanding of the evidence to support these roles, including impact, features of success and key challenges.

The key findings of the report are: 

• Where new roles have emerged, they have mainly done so due to developments in practice or to fill gaps in provision.
• There is a lack of evidence on the cost-effectiveness of new roles and the extent to which they improve patient outcomes.
• There are a number of ways in which integrated care is being delivered without the explicit need for new boundary-spanning roles.

House of Generations Schwaz: A one-stop shop for providing health and social services in Austria

The Municipality of Schwaz reorganized local health and social services around the purpose-built House of Generations; co-location of services increased coordination and improved access for vulnerable groups; shared living spaces, group activities and cooperative initiatives within the House of Generations generated a sense of community, fostering a supportive and inclusive environment for vulnerable groups; strong leadership helped forge effective partnerships with municipal and regional politicians through clearly communicating aims and objectives, contributing to the initiative’s successful development; extensive involvement and linkages with the wider community generated a new hub for community-based health and social services.

Highlights from ICIC19 in San Sebastian

The 19th International Conference on Integrated Care took place from 1-3 April 2019 in San Sebastian and was attended by 1400 delegates representing 60 countries.

With the overarching theme ‘Evaluating and implement models of integrated people-centred services’, the conference is a partnership of OSAKIDETZA – Basque Health Service, Bioef, Ministry for Health of the Basque Government the City Council of Donostia-San Sebastian and the Provincial Council of Gipuzkoa. The conference brought together researchers, clinicians and managers from around the world who are engaged in the design and delivery of integrated health and social care.

Can health and social care integration make long-term progress? Findings from key informant surveys of the integration Pioneers in England

All areas in England are expected by National Health Service (NHS) England to develop integrated care systems (ICSs) by April 2021. ICSs bring together primary, secondary and community health services, and involve local authorities and the voluntary sector. ICSs build on previous pilots, including the Integrated Care Pioneers in 25 areas from November 2013 to March 2018. This analysis tracks the Pioneers’ self-reported progress, and the facilitators and barriers to improve service coordination over three years, longer than previous evaluations in England. The paper aims to discuss these issues.

Enhanced Integrated Behavioral Health Model Improves Depressive Symptoms in a Low-Income, Uninsured, Primarily Hispanic Population Served by a Free and Charitable Clinic

Residents living in US-Mexico border communities have some of the worst health outcomes nationally. Rates of chronic disease such as obesity, diabetes, and related mortality in the Rio Grande Valley exceed those in most other regions of Texas and the nation. Poverty is pervasive, placing residents at high risk for poor health; they are more likely to be exposed to environmental hazards and have higher rates of chronic physical and mental health concerns.

At its foundation, integrated behavioral health care aims to address multiple health concerns, and related social determinants, by bringing behavioral health and primary care services together. No single model of integrated behavioral health care can be applied universally to health or social service settings; however, each approach is team-based and involves collaboration between multidisciplinary health and social service providers to achieve shared patient and community outcomes.

A Community Resource Navigator Model: Utilizing Student Volunteers to Integrate Health and Social Care in a Community Health Center Setting

While unmet social needs are major drivers of health outcomes, most health systems are not fully integrated with the social care sector to address them. 

This case study describes the development and implementation of a model utilizing student volunteer community resource navigators to help patients connect with community-based organizations. The authors then detail initial implementation outcomes and practical considerations for future work.

This white paper is part of the government’s commitment to transform the delivery of care in England following the Prime Minister’s announcement on reforms for health and social care through the Build Back Better: Our Plan for Health and Social Care.

This white paper is part of a wider set of mutually reinforcing reforms:

• the adult social care reform white paper
• the Health and Care Bill, and reforms to the health and care system

It sets out measures needed to make integrated health and social care a universal reality for everyone across England regardless of their condition and to level up regardless of where they live.

Though they have been in operation for several years in many parts of the country, integrated care systems (ICSs) across England are set to become new statutory bodies from July 2022. This will represent a significant shift in how health and care services are planned and delivered – away from the model of fragmentation and competition followed in previous decades (and reinforced through the Health and Social Care Act 2012), and towards one of collaboration between services.

Framework for countries to achieve an integrated continuum of long-term care

The framework for countries to achieve an integrated continuum of long-term care identifies key aspects necessary to achieve an integrated continuum of long-term care service provision and to facilitate the integration of long-term services within existing health and social care systems.

The framework will guide countries in assessing system-level components to implement sustainable and equitable long-term care actions. By applying this framework, countries can begin to develop and shape their long-term care systems as part of their universal health coverage programmes and promote investment in long-term care and the health workforce, including carers.

Factors and Mechanism Influencing Client Experience of Residential Integrated Health and Social Care for Older People: A Qualitative Model in Chinese Institutional Settings

An emerging service delivery model of integrating health and social care for older people has been actively promoted by the Chinese government since 2016, but the client experience and influencing mechanism still remain unclear. Methods: this study adopts a qualitative methodology to delve deeper into the factors and mechanism shaping the client experience of residential integrated health and social care for older people in the Chinese context, so as to understand the experiences of older residents during the whole process of receiving integrated care services, and on this basis, put forward suggestions for the improvement of a high-quality aged care service system. We coded and analyzed the in-depth interview data of twenty older adults and six staff members from June 2019 to February 2020, recruited from six institutions in Changsha, one of the ninety pilot cities for integrated health and social care in China.
